Not a day goes by without us helping someone who needs relief from the pain and suffering caused by a wisdom tooth.

The most common cause of dental pain for young adults aged between 18 and 28 is an infected wisdom tooth.


Wisdom teeth are the third molars or the 8th tooth at the end of the row of a full complement of teeth in all
4 corners of your mouth.


Wisdom teeth commonly get "stuck" while coming through the gum due to a lack of space.  Once they have partly "erupted" and are exposed to the oral environment, they can either give rise to a painful gum infection called "pericoronitis" or decay rapidly, causing harm to adjacent teeth. 


Such problems lead to the extraction of the wisdom teeth and extensive treatments of adjacent teeth.

It is therefore essential that you have your wisdom teeth regularly examined and X-rayed to prevent such problems.

We will examine your condition and take a panoramic X-ray to assess the risk of complications.
We will control the pain and treat the swelling which will require a prescription for antibiotics and strong pain-killers.  


The offending wisdom tooth (or teeth) usually need to be extracted about 7 days after your initial visit.
In the meantime we recommend that you rinse the affected area with warm salt water every day
to help eliminate the infection and reduce the swelling. 


In this practice, we routinely perform the removal of wisdom teeth for our patients while you sleep.
Our dentists will discuss the various options at your visit.




You will need to avoid eating for 4 hours before your visit and will need to have a responsible adult take you home
after the tablets have worn off.

FURTHER INFORMATION ABOUT WISDOM TEETH


                                                                               



Impacted wisdom teeth fall into one of several categories.


1. Mesio-angular impaction
is the most common form (44%), and means the tooth is angled forward, towards the front of the mouth and is jammed against the back of the second molar.


2. Vertical impaction (38%) occurs when the tooth is upright but there is insufficent space at the end of the row of teeth for it to fully come through the  gumline.


3. Disto-angular impaction (6%) means the tooth is angled backward, towards the rear of the mouth.


4. Horizontal impaction (3%) is the least common form, which occurs when the tooth is lying on its side, growing into the second molar at various levels, high, mid and deep.


Typically in the upper jaw, category 1 and 4 are difficult whilst categories 2 and 3 are easier.


In th lower jaw, category 1, 3 and 4  are very difficult and category 2 is generally easier.


Impacted wisdom teeth may be completely buried under the jawbone, in which case we do not recommend that they be removed.


If they are partially buried in bone or stuck under the soft gum at the back of the mouth, called the operculum, they are subject to causing unnecessary pain and suffering and must be removed. If left unattended, debris and bacteria can easily accumulate under the gum flap, causing infection in young adults which are often made more painful by contact from the opposing tooth. Common symptoms include a swelling and redness of the gum around the erupting tooth, difficulty in opening the mouth, a bad odor or taste in the mouth, and pain in the general area which may also run down the entire lower jaw, ear or possibly the neck. Untreated pericoronitis infection can progress to a much more severe infection as mentioned above, causing closure of the airways and potential death.


Wisdom teeth are extracted for two general reasons: either the wisdom teeth have already become impacted, or the wisdom teeth could potentially become problematic due to decay if not extracted. Potential problems caused by the presence of wisdom teeth even in a proper position include severe toothach due to deep decay in both the wisdom teeth and adjacent important second molars.  This is caused by food particles easily trapped on and around the wisdom teeth where regular brushing and flossing is difficult and ineffective. 


Other reasons wisdom teeth are removed include bad alignment which rubs up against the tongue or cheek causing pain, potential crowding of other teeth (a result of there being not enough room on the jaw/ in the mouth), as well as prior to orthodontics.


Many dentists and most oral surgeons recommend routine extraction of third molars (wisdom teeth) but in our practice,  we only recommend removal of wisdom teeth if one or more have PROVED THEMSELVES TO BE GUILTY  of causing problems.  As there is risk of injury to nerves in the area, although a very very low risk, WE DO NOT ROUTINELY RECOMMEND REMOVAL just because they are there! 


In our practice, we regularly perform the removal of "guilty" wisdom teeth using Oral Sedation, Nitrous Oxide Sedation and Local Anaesthesia. This is the most comfortable means of helping our patients, safe with the knowledge that you will not feel or remember any aspect of the treatment and with the medications we provide you, the recovery time will be minimal (usually 2-3 days) with mild pain and swelling. We recommend taking two days off following wisdom tooth surgery in the most severe cases.


Rarely, when high levels of risk to nerves in the area may be present, we refer patients to local Oral Maxillofacial Surgeons who may choose to perform the treatment under general anaesthesia in hospital. This is very rare, usually around 1% of the time.
